Vba Code worksheet with a list of 67 competencies and 3 criteria

Hello,

I have a worksheet with a list of 67 competencies and 3 criteria for each competency. A button is created on the same sheet such that upon clicking the button, a user form is created. I would like to obtain a vba code for this thank you.
