UT300R2U (ADSL) modem and a WRT54G v6 router

Hello,
I have a UT300R2U modem and a WRT54G v6 router. I can access internet using UT300R2U. However when I try to connect the modem to the router and the router to the computer, it does not work.

1) The setup CD asks me for a password (and not a username) and i have forgotten it. Also, hard resetting for 30 seconds does not work (even after restarting the router).
2) The http[colon]//192[dot]168[dot]1[dot]1, which is supposed to show a router setup page, throws a Page Can Not be Displayed error.
