I'm not sure if I understood well your question.
Do you want this formula works always at the last day of month?
Try to use it:
Your date --> A1
C1 -->
=IF(DAY(A1)=DAY(EOMONTH(A1,0)),"monthly activity - " & TEXT(A1,"MMMM"),"")
Is it what you want?

Try to use it:
