Batch moving files to mutiple folders

Hello,
Need a dos batch file to do the following task. (windows dos command)

I have a sets of txt files in the folder. 5th line of each file says "CustomerID,XXX" where "XXX" will be the destination folder for this file. The folder needs to be created if not available.
