No peoples isnt this the reason
here is the solution

open notepad
writhe this :
Windows Registry Editor Version 5.00

[HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Control\Storage­DevicePolicies]
"WriteProtect"=dword:00000000

after this click save as and save it usb.reg
important is to be .reg
save to desktop click on hem and try to format again

Configuration: Windows XP Internet Explorer 6.0
